Job Summary:
Serves as a single point of contact for referring physicians, patients and caregivers to provide resources and assistance with accessing clinical and supportive cancer care services offered within Kaiser Permanente, Colorado. This position assures the patient has the necessary appointments across the continuum for individual patient, including those made with lab, diagnostics areas and specialty physicians.
Essential Responsibilities:
- This position, knows and complies with all Kaiser Permanente quality, safety, and emergency policies and procedures. Demonstrates quality and effectiveness in work habits and clinical practice in every interaction with patients, colleagues, providers, and leadership. Ensures patient safety in the preparation and provisioning of care related to but not limited to medications including the consistent use of 5 rights and 3 checks of medication administration, procedures, infection prevention, fall prevention, including consistent use of two patient identifiers and procedural time outs. Reports safety hazards, accidents and incidents, and unsafe working conditions promptly.
- Assists patients in understanding their cancer diagnosis, treatment options, and the resources available. Responsibilities include assessment, analysis, planning and implementation of interventions.
- Serves as an essential link between patients and all other care providers in coordination of cancer care. The navigator facilitates changes in the plan of care and brokering necessary services i.e. inpatient, outpatient, home care needs, etc.
- Facilitate appointments for consultation and support services within the cancer care continuum including but not limited to cancer care counseling, dietitians, genetic & financial counselors, and supportive care. Coordinate and standardize patient cancer care education.
- Work with Cancer Registry staff and others to collect data, track outcomes, and support strategic planning processes. Help to support patient and their care giver(s) with clinical, emotional, spiritual, psychosocial, financial and other needs.
- Responsible for networking to maintain positive working relationships with stakeholders. Coordinate and facilitate tumor board(s) and multi-disciplinary clinics. Work collaboratively with sub-disease cancer care leads and committees and overarching cancer care council. Adhere to established productivity standards.
