Responsibilities:

• Perform patient assessment and assign diagnosis based on recorded assessment data and current scientific and current research.
• Establish plan of care and implement interventions in accordance with care paths, protocols, standards of care and current research.

• Develop and implement treatment plans.

Qualifications

Qualifications:

• A masters degree from an accredited APP program.
• Current or pending Illinois license as an APP.
• Current BLS and ACLS certification.
• Preferred - Minimum of one year of experience as an APP in a primary care setting preferred.
