Job Description

The Concurrent Measures Intervention RN (CMI RN) Team Leader (TL) provides day to day effective and coordinated leadership of the Concurrent Measures Intervention and Improvement Team, in conjunction with and in the absence of the Associate Director Quality Resources. CMI RN TL works to develop and improve team function and performance measures of program, division, and corporation, provides team performance feedback to Associate Director Quality Resources, and is responsible for ensuring staffs are mentored to provide quality interventions while promoting evidence based practices, according to measure guidelines. CMI RN TL reports on performance improvement measures progress, identifies barriers, and oversees implementation of improvements. CMI RN TL assists in providing fiscal management for the team.

The Concurrent Measures Intervention Registered Nurse (CMI RN), under the supervision of the Corporate Associate Director Quality Resources, functions independently overseeing measures of evidence-based care for a specialty group of patients, according to measure guidelines. The CMI RN is responsible for concurrent information and data collection, via interface with clinicians, the medical record, and through databases. The CMI RN influences care processes for designated measure patients in order to provide for evidence-based guideline care and outcomes, according to guidelines. The CMI RN provides real-time education to physicians, nursing, and support staff regarding appropriate screening, interventions and documentation to meet measure requirements; on-going education in relation to measure requirements; and facilitates patient education by ensuring appropriate patient education materials are available to the nurses on the units. The CMI RN is be responsible for staying current with all updates and changes in requirements, as well as reporting these changes to the appropriate reporting bodies. The CMI RN is responsible for data collection and submission of data to the vendor and/or Medicare intermediary, and reporting the results or findings, as appropriate to department leadership.

  • Bachelor of Science in Nursing
  • Minimum of 3 years nursing experience in acute hospital area
  • Core Measures experience
  • Knowledge of adult education/ teaching principles; working knowledge of Medicare reimbursement and coding structures. Experience in successful training of others to accomplish complex tasks
  • Familiar with evidence-based practice and related documentation 
  • Demonstrated leadership experience of influencing processes
  • Demonstrated ability to motivate and develop others through mentoring and guiding
  • Computer skills including Cerner Millennium, Microsoft Office (word processing, setting up spreadsheets, etc.)
  • Excellent communication and critical thinking skills; proficiency in managing conflict under stressful conditions; and ability to work independently in a time oriented environment
