Heart Failure Program Nurse (RN)

Position Overview:

Position is responsible for coordinating the continuity of care throughout a residents episode from acute care, to post acute care, and to home.  The heart failure nurse will work to cultivate trusting and synergistic relationships with each residents shared clinical partnership to ensure optimal outcomes.  

Respobsibilities:

  • Communicates directly with hospital and admissions team to begin gathering clinical requirements prior to admission to a skilled nursing facility
  • Performs pre-admission medical review with hospital care givers and skilled nursing center team to ensure safe transition from acute to post acute care center 
  • Rounds daily to evaluate residents clinical status 
  • Anticipates, identifies and solves problems efficiently with clinical expertise
  • Implements clinical interventions to maintain stability and improvement 
  • Coordinates weekly with mid level practicioner or physician to update on  residents current clinical status  through rounding, tele-health, phone or e-mail 
  • Documents proficiently on clinical occurrences and interventions 
  • Oversees and implements  new inititiatives or guidelines to prevent re-admissons and manage appropriate length of stay
  • Particpates on projects with acute and post acute providers to develop processes that improve resident outcomes
  • Collaborates with physician or mid level practicioner from multiple referral sources such as tertiary care centers, physician office, hospitals, home care agencies
  • Educates residents and families consistently on current overall plan of care or changes
  • Educates staff on specific skills required to care for residents affiliated with specialty program needs
  • Serves as the clinical coordinator  at the care center level to create a holistic approach with all disciplines involved with the residents plan of care 
  • Oversees the discharge plan to ensure a smooth transition from post acute to home with necessary additional care partners or families as appropriate

Qualifications:

Active, unencumbered CT RN license

Demonstrated proficiencies in the nursing process and clinical transitions.

Demonstrated experience in clinical standards and guidelines.

At least 5 years progressive responsibility in long term care and/or hospital setting.
