This is a hybrid remote work from home position, following orientation, compliance, and training episodes. Join a healthcare team offering an innovative approach to clinical management and patient support while providing 24 hr. / 7 day a week remote monitoring and interactive care. You will provide clinical at home assessments and tasks to patients within Allegheny County.

Support nonclinical work as needed, when working within the physical office location. Current initiatives for remote monitoring of HF, COPD, and PPHTN patients.

You will work primary 7:00 am – 7:30 pm (8-hour shift within these hours, will be primary schedule) Yes, flexible with scheduling while meeting the needs of the program and patients. Weekend rotations every 2 to 3 weekends. May require occasional overnight rotation, which would be work from home for this shift. Orientation will begin with daylight hours, and then transition to off shift hours, as needed. Sign-on bonus available for those that qualify.

Responsibilities

• Patient care and remote interactions, while supporting a patient’s clinically safe health status within their home environment.
• Follow escalation protocols for patient care in a home environment
• Support In Home Medic team program with patient clinical task and assessments when needed.

• Comfortable with change, innovation, and strategic thinking.
• One (1) year Medical/Surgical experience in an acute care facility and/or skilled nursing facility required. Two (2) years medical/surgical experience in an acute care facility and/or skilled nursing facility, preferred.
• Graduate of an accredited school for practical nursing - Required

• Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)
• Driver's License
• Act 34
• Licensed Practical Nurse (LPN) OR Temporary Practice Permit (TPP)
