The Nephrology midlevel provider will be an integral member in establishing and managing a panel of Veterans assessed as needing nephrology care and follow-up visits, managing Veteran's needs for follow up (including imaging, tests, and clinic visits), managing consults, managing referrals for nephrology treatment, follow up with Veterans who have received their nephrology care in the community by assuring these Veterans are managed appropriately and not lost to follow up.
