Job Description:

As a Remote LPN/LVN at Circadia Health, you will play a pivotal role in remotely monitoring and reviewing the vital signs and EHR data of patients in Skilled Nursing Facilities using our advanced remote monitoring system. Under the supervision and guidance of an RN overseeing the team, your expertise and attention to detail will contribute to the early detection of changes in patient condition, enabling timely intervention and improved patient care. **Certified/Licensed as a Licensed Practical Nurse or Licensed Vocational Nurse in one or more of the following states: California.

Responsibilities:

  • Remote Patient Monitoring: Review vital signs and EHR data of patients in Skilled Nursing Facilities using our FDA-Cleared contactless patient remote monitoring system.
  • Patient Condition Documentation: Document changes in patient condition based on the observed trends in vital signs data and EHR information, under the supervision of an RN.
  • Risk Assessment Collaboration: Collaborate with the RN overseeing the team to help define risk levels of patients based on the remote monitoring data.
  • Digital Reporting: Develop digital reports for clinicians at Skilled Nursing Facilities, providing valuable insights and data for informed decision-making.
  • Remote Collaboration: Work remotely with our healthcare team, including the RN, product managers, clinical implementation specialists, to ensure optimal patient care and monitoring.
  • Adherence to Regulations: Comply with all relevant healthcare regulations, ethical guidelines, and company policies while handling patient data.
  • Ability to work in a fast paced environment completing multiple tasks--startup mentality.
  • Experience working as an LPN or LVN
  • Preferred experience working with EHR (e.g. PointClickCare)
  • Excellent critical and analytical thinking skills
  • Strong verbal and written communication skills
  • Familiarity with medical terminology and clinical decision support.
  • Preferred experience using GSuite: Gmail, Docs, Sheets, Slides
  • Team spirit with a positive attitude.
  • Ability to listen, comprehend, and process a variety of detailed information
  • Willingness to continue gaining knowledge and clinical experience.
  • Certified/Licensed as a Licensed Practical Nurse or Licensed Vocational Nurse in one or more of the following states: New York, Connecticut and/or Compact.
